Undergraduate Student Health Insurance

Utah State University offers an insurance policy through First Underwriters . The plan is underwritten by The Chesapeake Life insurance company.

If you were covered last Summer and have paid premiums for Fall Semester by the enrollment deadline, existing covered conditions will roll over with continuous coverage. Brochures, including applications and Insurance/RX (prescription) cards are available at the Student Health and Wellness Center, 850 East 1200 North.

All insured students are required to use the Student Health Center first, before going to a physician elsewhere (off campus). You will need a referral to an off-campus physician, so you must come to the Student Health Center first.

However, if you have an emergency and/or the Student Health Center is closed, please go to Logan Regional Hospital Clinic or Emergency Room located at 500 E. 1300 N.

The Insurance plan has NO DEDUCTIBLES! You will be required to pay a co-pay (co-pay amount depends on the type of services received off campus, i.e. Office visits $ 30.00 co -pay, Emergency Room visits $250.00 co-pay, etc.) PLUS 30% of the balance for services received downtown (off campus) when you use the IHC/Beechstreet Provider Network of doctors and hospitals. A list of providers is available online through Beechstreet.

Prescriptions rendered "medically necessary" will require a $15.00 co-pay for generic medications and a $30.00 co-pay for name brand medications. There is a maximum coverage of $500.00 per year. (The $500.00 limit is based on the full price of the medication and not the co-pay amount).

No Pre-existing condition will be covered until you have been continuously insured by the plan for twelve months. A pre-existing condition is defined as any accidental injury or sickness for which you have received medical treatment, services, medication, or advice, or which was diagnosed in the twelve months preceding the effective date of your coverage. Routine dental, optical and elective procedures such as "check-ups" are not covered. Please note "exclusions" in the brochure.

Student Health Services has a gynecological service for annual exams. (This includes routine physical and pap smear only). Covered benefits include care provided by hospitals, specialists, ambulance, prescription, maternity, surgery and other medically necessary care downtown (off campus) as defined in the brochure.

Electronic Paperwork (EPAF) for qualifying assistantship or fellowship (notify Financial Aid office) is completed by the department (Deadline: August 20th) If there are mitigating circumstances that are preventing the department from completing this paperwork, the department should alert Noell Hansen in Student Health and Wellness Center with the Student's Name, A#, Birth Date, Email Address, EPAF Begin/End Date, Account Code, and FTE. If a qualifying through a $10,000 or more fellowship/scholarship, the department should email the Student's Name, A#, Birth Date, Email Address, Funding Begin/End Date, and funding amount. Noell Hansen will assess the student premium of $237 and remove any international insurance fees (Approximate Deadline: August 24th-August 28th) If you are entitled to a waiver, but must pay the premium in order to obtain your classes, you WILL receive a full refund of the waived amount and only pay the lower (subsidized) premium. Students who have enrolled on the website by or before September 16th will be enrolled in the insurance program with an effective date as of August 20th and the insurance company will mail them a packet and permanent insurance card. Students who have waived through the website by September 5th should have their student premium removed from their account and will be added to the "approved hard waiver list" that will be sent to every department on a weekly basis.
